Use of Building Request Form

PLEASE NOTE:
This form must be submitted at least one month PRIOR to the requested event.  This is to allow arrangements for a Building Coordinator, A/V needs and set up.  Exceptions may be possible, but less than one month lead time may result in non-approval of the event.  Note:  non-church functions must not interfere with regularly-scheduled church functions, such as worship services, Bible studies, etc.







It is therefore agreed by and between the parties:
1.    Person responsible agrees that it will not use the premises for any unlawful purposes, and will obey all laws, rules and regulations of all governmental authorities while using the described premises.
2.    Person responsible agrees that it will not use the premises for any purpose that is contrary to the mission, purpose or belief of Christ Our Savior Lutheran Church, which is a biblically-based religious institution.
3.    Person responsible agrees to abide by any rules or regulations for the use of the premises that are attached to this agreement.
4.    Person responsible agrees that it is solely responsible to implement appropriate screening and supervision procedures to protect children, youth, and vulnerable adults attending function at Christ Our Savior Lutheran Church.
5.    Person responsible agrees to hold harmless, indemnify and defend Christ Our Savior Lutheran Church (including employees and representatives) from any and all liability for injury or damage including, but not limited to, illness, exposure to infectious/communicable disease, bodily injury, personal injury, emotional injury, or property damage which may result from any person using the described premises, its entrances and exits, and surrounding areas, for this purpose.
6.    Person responsible agrees to be responsible for preparing for use and returning to the pre-use condition all areas of the premises which will be used, including entrances and exits.

Fees are evaluated carefully for each event by our building Coordinator and COS Staff. Please be aware that the following fees MAY apply to your event. You will be contacted after the building request has been reviewed with final information about any fees before they are assessed.

Fees (if applicable): Building Coordinator fee -- $25 for 1st hour and $15 each hour thereafter
Cleaning fee -- varies
